29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in the operator's view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement. Establishment: The parking brake of the forklift was not set to prevent movement of the forklift when the operator was dismounted from it. On or about January 4, 2023, an employee sustained crushing injuries after his foot was run over by the wheel of an unattended forklift.
