VEHICLES MANEUVERING IN WORK AREAS :
a)CO-MINGLE BIN, on or about 05-21-97: An employee who was walking
and/or working in an area while multiple vehicles were in use, was run over
by a truck that was backing up to unload recyclables.
b)TIPPING FLOOR, on or about 05/22/97: An employee was operating a Case
Model W20C Front End Loader without a horn and a backup alarm.
ABATEMENT NOTES: Among other methods, one feasible and acceptable
abatement method to correct this hazard is to:
1)Repair and/or replace the horn on front end loader, and
2)Install an audible backup alarm on the front end loader,
and
3)Develop and implement procedures to ensure employees
are not in the danger zone where vehicles are
maneuvering.
