were causing or likely to cause death or serious physical harm to

were exposed to being thrown from the operator's compartment or being
struck by loader:
a)At the facility, on or about 3/15/02:  Employee operating a Case 1845C
Uni-
Loader, was not wearing the seatbelt provided.
b)At the facility, on or about 3/15/02:  Employee operating a Case 1845C
Uni-
Loader and the audible backup alarm was not functioning.
Among other methods, some feasible and acceptable methods to correct this
hazard is to:
(1)  Ensure that the loader is in good operating condition and that all its
safety equipment is correctly installed and utilized;
(2)  Ensure that every operator is familiar with the safety equipment to
include its function and purpose; and
(3)  Instruct operators to report malfunctions or unsafe conditions prior
to
operation.
