Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards in that employees were exposed to the hazard of being struck by a falling load or monorail crane framework collapse as a result of the crane structure failing: a. Flatbed Truck Referred to as "The Beast" - A truck mounted monorail crane had been installed, erected and used for tasks including, but not limited to, lifting and transporting concrete vessels where the unit did not have the proper engineering ratings or approvals and did not have proper inspections performed. ABATEMENT NOTE: One feasible and acceptable abatement method, among others, to correct this hazard is to install and operate a crane and associated equipment in accordance with the American National Standards Institute guidelines such as, but not limited to: ASME/ANSI B30.11-1988 Monorails and Underhung Cranes ASME/ANSI B30.20-1985 Below-the-hook Lifting Devices ANSI/AWS D14.1-85 Specification for Welding of Industrial and Mill Cranes and Other Material Handling Equipment
