Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to live electrical parts:
a.)M/V New West: In the Wet Room, a live white extension cord hanging from
the ceiling
exposed employees to live electrical parts.
Among other methods, feasible and acceptable methods of abating this
hazard would be
to remove the extension cord or install hard wiring for this type of
environment.
b.)M/V New West: In the Galley, a two-prong receptacle was not
GFCI-protected at either
the receptacle or the panel.
Among other methods, one feasible and acceptable method of abating this
hazard would
be to install a GFCI at the receptacle or the panel.
NOTE:  New West Fisheries, Inc. was previously cited for violations of this
Occupational Safety and Health Act requirement in OSHA inspection
#107106312,
citation 1, item 1, issued on 8/13/97, with respect to a workplace located
at Naknek,
Alaska; and OSHA inspection #301494993, citation 1, item 1, issued on
9/21/98, with
respect to workplace located in Naknek, Alaska.
Abatement Note: In addition to submitting the enclosed Abatement
Certification form to
OSHA, abatement documentation in the form of photographic evidence must be
provided.
Failure to provide such documentation may result in additional citations
and penalties.
