Section 5(a)(1) of the Occupational and Health act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to the unexpected release of reels of cable due to improper installation of winches for lifting applications and inadequate inspections: LIQUID FILL: WINCHES WERE INSTALLED ON LINES, 1, 2, 3, 4, 5, 6, AND 8 TO LIFT PUMPS AND AIR MOTORS. 1.AS PER THE INFORMATION PROVIDED IN THE OWNERS MANUAL THEY WERE NOT SUITABLE FOR LIFTING APPLICATIONS. 2.THE WIRE ROPE ON FILLING LINE SIX HAD BEEN REPLACED WITH A COTTON CLOTHES LINE TYPE OF ROPE. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD, AMONG OTHERS IS TO: 1.REPLACE THE WINCHES WITH HOIST THAT WERE DESIGNED AND APPROVED FOR LIFTING. 2DEVELOP AND IMPLEMENT A SCHEDULE OF REGULAR, DOCUMENTED INSPECTIONS FOR LIFTING EQUIPMENT.
