Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being punctured:
a)Recycling building, south side, C100 and C101 conveyor belt - Employees
with
the job title of picker who work around the C100/C101 belts were not
provided
with an emergency stop device to stop the conveyor equipment in the event
of an
emergency.  On or about 03/03/98.
ABATEMENT VERIFICATION IS NOT REQUIRED AS OUTLINED IN PART 29 CFR
1903.19
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Install emergency stop devices along conveyor belts in accordance with
ANSI/ASME
B20.1-1984, and
2.Train employees on the hazards associated with working around the
conveyor belts and
trommel, the necessary precautions to take to prevent injury, and the
proper use of safety
devices.

employees in that employees
were exposed to the hazard of being thrown and/or crushed:
a)Worksite - The seat bar safety interlocking systems in two Bobcat 943
skid steer
loaders (s/n 503313283 and 503313315) did not lock the lift and tilt
control foot
pedals in neutral when the seat bar was in the elevated position due to
the fact
that the spring loaded interlock latches were not functioning.  The system
was not
serviced.  On or about 06/08/98.
ABATEMENT VERIFICATION IS REQUIRED AS OUTLINED IN PART 29 CFR 1903.19
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Ensure that all safety items, including, but not limited to, the seat
bar safety
interlocking system and lift and tilt control interlock latch are fully
operational
before use of skid steer loaders, and
2.Train employees in the hazards associated with the improper operation of
skid
steer loaders and the proper application of safety devices.

were exposed to falls from elevations:
a)Worksite - Employees are elevated to heights of greater than 10 feet on
a wooden
pallet or trash bale suspended on the forks of a forklift truck without
the
use of
fall protection.  During the period between 2/1/98 and 3/1/98.
ABATEMENT VERIFICATION IS REQUIRED AS OUTLINED IN PART 29 CFR 1903.19
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Utilize only platforms designed for elevating personnel with forklifts.
These platforms
must meet the requirements set forth in ANSI/ASME B56.1-1993 such as, but
not limited
to, fall protection for employees.
2.Train all employees about the hazards of falls associated with improper
elevating methods
and proper application and use of safety platforms.
