Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)  the hazards of falls in that employee(s) climbed onto the basket
guardrail of a boom
supported elevating work platform to access the platform by the gear box
for the 15-ton crane.
American National Standards Institute A 92.5-1992 Section 7.11.1 prohibits
the use of any object
to gain additional height on such equipment.  Among other feasible and
useful methods of
abatement, one method would be to install and use fixed ladders with grab
bars to access the
platform.
