Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Monfort Beef Division, Hwy. 287 North, Cactus, TX. For the period of time up to and including 10/22/99, fork lift operators were observed driving fork lifts in the loading dock area without wearing the safety belts provided. This exposed employees to the hazards of being struck / crushed by the lift truck overhead protection device should the fork lift be accidentally tipped over and result in the driver being thrown out of the operator seat. A feasible and acceptable abatement method to correct this hazard would be: Ensure that drivers wear supplied safety belts anytime they operate an industrial fork lift truck.rial

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Monfort Beef Division, Hwy. 287 North, Cactus, TX. For the period of time up to and including 10/22/99, the employer did not maintain pressure and temperature gauges in an operable condition throughout the facility. Gauges that would alert employees working in specific hazardous areas of the facility were pegged (indicating that over pressure or over temperature occurred), corroded, damaged or unreadable. These gauges give maintenance personnel information while in the facility and away from the main computer controls. This exposed employees to the hazards and injuries related to the over charging of lines or the sudden release of ammonia. Instances: Note that the instance below represent a sampling, similar conditions with gauges exist throughout the facility. a) pressure gauge on boiler #5, glass or plastic cover missing and no indicator needle. b) pressure gauge in boiler room, glass or plastic cover missing. c) pressure gauge on compressor #2, glass or plastic cover missing. d) pressure gauge on boiler #2, glass or plastic cover is broken, gauge not readable. e) pressure gauge on boiler #5, glass or plastic cover missing. f) pressure gauge on boiler #3, glass or plastic cover missing. g) gauge #169, difficult to read and gauge is "pegged". h) temperature/pressure gauge on booster #7 (near check valve #339), defective. i) gauge #231, unreadable. j) temperature/pressure gauge on unit #4 (compressor room), indicator needle missing. k) temperature gauge on unit #4 (compressor room), gauge is unreadable. l) temperature/pressure gauge (box area), glass or plastic cover missing. m) temperature/pressure gauge #1479 (Fab area catwalk), glass or plastic cover missing, rusted and inoperable. n) pressure gauge on boiler #4, glass or plastic cover broken.
