Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees. Employees were in self propelled extensible boom aerial lift without any fall arrest. Among feasible means of abatement would be to comply with the American National Standards Institute, ANSI A92.5-1992 Standard for Boom-supported Elevating Work Platforms and, 1.Have the employee wear fall arrest anytime that they are in the aerial lift. 26 feet above grade - Employees were riding down in an aerial lift without fall protection at the Warrenville jobsite. Fall protection was not used. NO DOCUMENTATION REQUIRED FOR THIS ITEM.

Among feasible means of abatement - comply with the Manufacturers recommendations and/or the American National Standards Institute, ANSI A92.5-1992 Standard for Boom- support Elevating Work Platforms. 26 feet above grade - Employees riding in an overloaded aerial lift at the Warrenville jobsite were exposed to fall/struck by hazards. Load capacity limits were not followed. NO CERTIFICATION REQUIRED FOR THIS ITEM.
