Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious bodily harm to
employees in that
employees were exposed to the hazards of unexpected energization or start
up of machines or
equipment or release of stored energy.
On or about 12-05-00, inside of the after steering compartment of the tow
boat Larry Brock,
the employer failed to ensure that the hydraulic tiller rams attached to
the rudder yokes had
been de-energized and locked out before employee exposure occurred.
One useful method of to correct this apparent hazard would be to implement
a lockout/tagout
program for the control of hazardous energy.
