Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist below-the-hook
lifting devices fail or should the wire rope on the winches fail:
a)On or about 10/16/97 through 10/24/97, the following overhead hoists had
safety latches missing from the hooks, roller chain was kinked and had not
been inspected, including, but not limited to:
1)Mould Shop, four Yale 1/8-ton overhead hoists: Hoists #1 and #2 had
no safety latch on the lifting hook, and no inspections performed; hoist
#3 had the safety latch on the lifting hook taped back, no safety latch
at the hook attaching the hoist to the trolley and no inspections
performed; and hoist #4 had a broken safety latch at the lifting hook,
no safety latch at the hook attaching the hoist to the trolley, the roller
chain was kinked in one area, and no inspections performed.
2)Clay Prep, Cellar #1, northeast end: Budgit one-ton overhead hoist
had no safety latch on the lifting hook, no safety latch at the hook
attaching the hoist to the trolley and no inspections performed.
3)Clay Prep, Cellar #2, at E & J Blunger #1: CM Lodestar two-ton
overhead hoist had no safety latch on the lifting hook and no
inspections performed.
4)Clay Prep, Cellar #2, northeast corner, Blunger #4: CM Valustar two-
ton overhead hoist had no safety latch on the lifting hook and no
inspections performed.
5)Department 44, Forklift Maintenance: Coffing overhead hoist had no
safety latch on the lifting hook and no inspections performed.
6)Spray Glaze, Batching Platform, west side: CM Lodestar overhead
hoist had no safety latch on the lifting hook and no inspections
performed.
7)Machine Shop: Budgit 1/8-ton overhead hoist had no safety latch on
the lifting hook, no safety latch at the hook attaching the hoist to the
trolley and no inspections performed.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazard are to:
1.Install safety latches on the lifting hooks and hooks attaching the hoist
to the trolley.2.Repair and/or replace the roller chain to eliminate the
kink.
3.Conduct frequent and periodic inspections in accordance with ANSI
B30.16-1987, Chapter 16-2.
b)On or about 10/16/97 through 10/24/97, the following below-the-hook
lifting
devices had no rated load markings, and no inspections performed,
including,
but not limited to:
1)Clay Prep, northeast corner of Cellar #2, #4 Blunger: A Bethlehem
spreader beam attached to a CM Valustar overhead hoist had no rated
load capacity markings and no inspections performed.
2)Clay Prep, northwest side of #4 Cellar: A spreader beam used for
lifting scrap into the scrap feeder attached to a one-ton Budgit
overhead hoist had no rated load capacity markings and no inspections
performed.
Among other methods, several feasible and acceptable abatement methods to
correct
each of these hazards are to:
1.Mark the rated load of the lifting device on the main structure or
equivalent.
2.Conduct frequent and periodic inspections of the lifting devices in
accordance with ASME B30.20-1993, Chapter 20-1.
c)On or about 10/16/97 through 10/24/97, the following winches had damaged
wire rope, improper rigging of the wire rope to the hooks, welded
attachments to the hooks and no inspections conducted, including, but not
limited to:
1)Bisc Kiln #3, Thern hand winch: Wire rope was birdcaged in several
areas, had several broken strands, and no inspections were conducted.
2)Bisc Kiln #3, west side, Post O-13: Braden electric winch had
improper rigging of the wire rope to the hook, the hook had welded
attachments, and no inspections were conducted.
3)Bisc Kiln #3, northwest entrance, Braden electric winch: Wire rope
was birdcaged, there was a knot in the wire rope, there was improper
rigging of the wire rope to the hook, and no inspections were
conducted.
4)Bisc Kiln #1, northwest entrance, Broken Arrow electric winch: The
wire rope had numerous broken stands and no inspections were
conducted.
5)Bisc Kiln #1, west side, back row, Jeamar electric winch; Wire rope
was birdcaged, there was improper rigging of the wire rope to the
hook and no inspections were conducted.
Among other methods, several feasible and acceptable abatement methods to
correct
each of these hazards are to:
1.Replace wire rope that is birdcaged and has numerous broken strands.
2.Properly attach the U-Bolt clips to the wire rope so that the U-Bolt of
the clips is on the dead end of the rope and the proper number of U-
Bolts clips are used as to the size of the wire rope.
3.Conduct frequent and periodic inspections of the hooks in accordance
with ANSI B30.10.
4.Conduct frequent and periodic inspections of the winches in
accordance with ASME B30.7 and the manufacturer's
recommendations.
Abatement documentation must be submitted for this Citation.
