Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, monorail and/or below-the-hook lifting devices fail, not function properly or should the load slip off the hook from lack of safety latches: a)On or about 12/13/01 through 1/30/02, the following overhead hoists had broken safety latches on the lifting hooks and/or the lower limit switches were not functioning: 1.Mold Shop, Yale 1/8 ton overhead hoist located at table #3 had a broken safety latch on the lifting hook. 2.Mold Shop, Yale 1/8 ton overhead hoist located between tables #1 and #2 had a broken safety latch on the lifting hook. 3.Clay Prep, Cellar #1, Coffing one ton overhead hoist located at scrap feeder #2 had a broken safety latch on the lifting hook. 4.Clay Prep, Cellar #2, Coffing two ton overhead hoist located at scrap feeder #2 had a missing safety latch on the lifting hook and the lower limit switch was not functioning. 5.Spray Glaze Dept., Glaze Making Batching Platform, CM Lodestar 1/8 ton overhead hoist had a missing safety latch at the hook attaching the hoist to the trolley. 6.Fire Power Kiln Car Repair, Coffing #30 three ton overhead hoist, the lower limit switch was not functioning. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Install new safety latches on the lifting hooks in accordance with ANSI B30.16-1993 Section 16-1.2.9. 2.Adjust the lower limit switches in accordance with ANSI B30.16-1993 Section 16-2.3.3. b)On or about 12/13/01 through 1/30/02, the following below-the-hook lifting devices had no rated load capacity markings: 1.Clay Prep, Cellar #2, Scrap Feeder #2, a Bethlehem spreader beam attached to a Coffing two ton overhead hoist had no rated load capacity markings.2.Clay Prep, Cellar #4, Scrap Feeder #7, a spreader beam attached to a Coffing two ton overhead hoist had no rated load capacity markings. 3.Fire Power Car Repair Station, a spreader beam attached to a Coffing three ton overhead hoist had no rated load capacity markings. Among other methods, one feasible and acceptable abatement method to correct hazard is to: 1.Mark the rated load of the lifting device on the main structure or equivalent in accordance with ANSI B30.20-1993 Section 20-1.2.1. c)On or about 12/13/01 through 1/30/02, the following monorails had no rated load capacity markings on them: 1.Spray Glaze Dept., Glaze Making Batching Platform, a monorail with a CM Lodestar 1/8 ton overhead hoist #32 attached, had no rated load capacity markings on the rail. 2.Mold Shop, a monorail with three Yale 1/8 ton overhead hoists and one CM Lodestar overhead hoist attached, had no rated load capacity markings on the rail. 3.Clay Prep, Cellar #1, a monorail with a Coffing #17 one ton overhead hoist attached, had no rated load capacity markings on the rail. 4.Clay Prep, Cellar #2, a monorail with a Coffing #23 two ton overhead hoist attached, had no rated load capacity markings on the rail. 5.Clay Prep, Cellar #4, a monorail with a Coffing #24 two ton overhead hoist attached and a monorail with a Coffing #25 one ton overhead hoist attached, had no rated load capacity markings on the rails. 6.Maintenance Shop, a monorail with a Coffing #28 one ton overhead hoist attached, had no rated load capacity markings on the rail. 7.Machine Shop, a monorail with a CM Lodestar #28 one ton overhead hoist attached, had no rated load capacity markings on the rail. 8.Power House, a monorail with a Dayton 1/2 ton overhead hoist attached had no rated load capacity markings on the rail. 9.Forklift Repair, a monorail with a Coffing #33 three ton overhead hoist attached, had no rated load capacity markings on the rail. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1.Mark the rated load on the main structure in accordance with ANSI B30.11-1980 Section 11-1.1.2. Abatement certification must be submitted for items b-1, and b-2.
