Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of contusions and lacerations. (a)M/V American Enterprise, Baader area: At electrical power panel LF-3, the employer failed to ensure that the working space in front of the panel was clear and unobstructed to prevent a potential injury to an employee required to work in the space during an emergency response to disengage the power supply. Note: One feasible method of abatement, among others, is to clear the working space in front on the panel and not use it for storage.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrical shock and burns, where the potential use of a damaged extension cord with an ungrounded or defective power tool could occur. (a)M/V American Enterprise, engine room, port side bulkhead: A yellow extension cord was hanging from a hand rail. The three-way connector (female end) of the cord was separated from the outer insulation which serves as the strain relief for the inner conductors to prevent them from breaking off of or asserting pressure directly to the terminal screws or connection points. Any/all damaged portable power equipment, to include extension cords, must be repaired before use. Note: One feasible method of abatement, among others, is to replace the female connector, then test the cord for proper polarity and or a short before use.

Note: One feasible abatement method, among others, is to replace the batteries.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of muscle sprain,strain or contusions and/or lacerations when working over a damaged conveyor belt: (a)M/V American Enterprise, factory area: Where the conveyor belt delivers packed product to the freezers, the employer failed to have in place an effective inspection/maintenance program to insure that the conveyor system/components were maintained in a condition which would prevent a hazard to employees. (b)M/V American Enterprise, factory area: The conveyor belt that delivers packed product to the freezers contained a hole (3-3/4 inch by 5 1/4-inch) into which an employee could accidently insert his hand when placing or removing materials on the belt. Employee's hand could also be caught in the hole during operation and rip/tear could also snag an employees hand/fingers during operation. Note: One feasible means of abatement, among others, is to repair the belt by gluing/sewing a cover of like fabric over the hole and to stitch/sew the rip together with heavy duty thread. Suggestion: The employer should develop a written inspection/maintenance program to insure that all conveyor components are maintained free of hazardous conditions.
