Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to electrocution. The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release or stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): (a)M/V Kodiak Enterprise: When employees perform service on electrical powered factory equipment utilized to process seafood, the employer failed to establish, or to have available a written plan for the energy control program as required in 1910.147. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop a written energy control plan.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrocution because the employer's tagout energy control program did not comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection: (a)M/V Kodiak Enterprise: When employees were working on the hydraulic oil purifyer the electrical controls were not locked-out and the tag-out utilized was not effective. Note: Among other methods, one feasible and acceptable abatement method would be to develop effective lockout/tagout procedures.
