Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to electrical burns or electrocution where the potential use
of an ungrounded or
defective power tool or appliance could occur:
The following conditions contributed to the existence of this hazard:
(a)M/V Independence:  In the AFT/FWD boiler room where an employee had used
an extension cord in the process of his work, the employer failed to
ensure that
appropriate removal of the extension cord was conducted upon completion of
the
work. The cord was still connected to the outlet and the female end of the
cord
was laying in water which created a short in the circuitry and subjected
employees to the hazard of electrical burns or electrocution.
Among other methods, one feasible and acceptable method to correct this
hazard
is to disconnect, remove and properly store the extension cord after each
use.

while using a
defectively wired receptacle as a power source for electrical
tools/appliances:
(a)M/V Independence, engineers tool room:  An electrical outlet, mounted
on an
overhead beam and on the bulkhead, was improperly wired and indicated
reverse
polarity when tested.
Note: One feasible method, among others, is to reconnect the wiring to the
appropriate terminals and test the outlet for correct reading before use.

were exposed to the hazard of falling to a lower level due to an unguarded
opening:
(a)M/V Independence:  The employer failed to provide complete operational
doors
or gates with interlocking devices on the port forward freight elevator
and the
starboard forward freight elevator to prevent the elevators from being
operated
while any door or gate is opened. Employees could fall into the open
elevator
shaft if the elevator was not stopped at the platform or be struck by the
moving
car.
Among other methods, one feasible and acceptable method of abatement would
be the installation of either a mechanical or electrical interlocking
device to
prevent operations of the car with an open front.
