inspection/certification
of energy control procedures was incomplete and did not contain the
required
documentation as to the identification of the machines/equipment,
inspection date,
name of affected employees and/or name of the employee performing the
inspection.  There was no written documentation to ensure that the
procedures/requirements of 1910.147 are being followed.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to develop and implement a written lockout/tagout
program.
