Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of electrical shock, burns and/or electrocution
where exposure to the
potential use of ungrounded or defective portable power tools and/or the
use of appliances in
wet/damp locations are used in combination with incorrectly wired
electrical outlets and/or
receptacles without a GFCI.  The following conditions contributed to the
existence of these
hazards:
(a)F/T Northern Hawk - On the port side of the upper engine room, two
receptacles
indicated reverse when tested with IDEAL circuit tester #61-035.
(b)F/T Northern Hawk - In the upper engine room, one receptacle indicated
a short
circuit when tested.
(c)F/T Northern Hawk - In the galley area, a receptacle at the rinse sink
and a
receptacle over the countertop at the rear portion of the galley indicated
reverse
polarity when tested and neither receptacle was suitable for the location
as they
were not of the GFCI type.
(d)F/T Northern Hawk - In suite #330, the receptacle located over the desk
indicated
a short circuit when tested.

5(a)(1) of the Occupational Safety and Health Act of 1970: The employer
did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or were likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of electrical shock, burns or trauma where
exposure to the potential
use of ungrounded appliances in areas that are wet or damp could occur.
The following
conditions contributed to the existence of these hazards:
(a)F/T Northern Hawk, in the engine room - A portable quartz light fixture
had a
damaged/defective service cord. The plug was missing its ground pin and
the cord
conductors appear to have been spliced and the splice was wrapped with blue
tape. The employer failed to attach a "Danger Do Not Use Tag" and/or
failed to
remove the damaged/defective fixture from service until proper repairs
could be
made.
(b)F/T Northern Hawk, in the engine room - Where an extension cord used as
a
power source for the graymil mineral spirits vat contained damage to the
outer
insulation at the connector which prevented proper strain relief of the
conductors
at the connectors terminals, the employer failed to remove the
damaged/defective
extension cord from service until proper repairs could be made.
