Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause serious physical harm to employees in that employees were
exposed to the hazard of amputation and/or severe laceration during
blade changes and cleaning operations involving header and gulleter
machines from accidental start-up reactivation:
F/V Snopac, processing area - Where engineering/processing
personnel were required to clean and maintain each of the header
and gulleter machines at this location, workers exposed themselves
to the hazard of severe laceration and/or amputation from contact
with sharpened blades energized by electrical and pneumatic
sources. The energy isolation program in place consisted only of
generic verbiage and did not direct the user of the procedure with
specific steps for shutting down and isolating equipment nor did it
cover the placement, removal, and transfer of lockout/tagout
devices; machine specific procedures were not developed for testing
verifying the effectiveness of these devices and other control
measures such as pressure bleed and valving.
ABATEMENT NOTE: Among other methods, one feasible and acceptable
abatement method to correct this hazard would be to develop and
implement equipment specific lockout/tagout procedures for these
machines

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause physical harm to employees in that employees were exposed
to inhalation as well as fire/explosion hazards as a result of
severe corrosion on the manifold assembly of stored ammonia
cylinders and the potential for these pressurized cylinders to
leak:
F/V Snopac, deck storage area - Where ammonia cylinders were stored
in such a manner that they were exposed to the effects of weather
and salt water, the manifold of one of the cylinders (full) was
severely corroded and posed the potential for the contents to leak;
a leaking cylinder in this location would have created an
inhalation hazard to personnel in the vicinity of the cylinder as
well as a reentrainment hazard to occupied spaces within the
vessel.
ABATEMENT NOTE: Among other methods, one feasible and acceptable
abatement method to correct this hazardous condition would be to
store anhydrous ammonia cylinders in a protected location and
inspect these cylinders periodically (for instance quarterly) for
indications of corrosion.s
