furnish to each of his employees conditions of employment and a
place of employment which were free from recognized hazards that
were causing or likely to cause death or serious physical harm to
employees:
a)facility wide--below the hook lifting devices, such as
the one used in the slitter department and the demag
hoist used in the annealing department were not provided
with;

Safety and Health Act of North Carolina:  The employer did not
furnish to each of his employees conditions of employment and a
place of employment which were free from recognized hazards that
were causing or likely to cause death or serious physical harm to
employees:
a)facility wide--below the hook lifting devices, such as
the one used in the slitter department, were not provided
with an operational test prior to their initial use.
REFERENCE:ANSI B30.20-1985 Chapter 20-1.4, TESTING

modified, or repaired lifting devices shall be
tested to ensure compliance with this
Standard.
NOTE:Among other methods, one feasible and acceptable
method to correct this hazard is to have all below

Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees:
a)sheet metal department--a support structure used to support for both a 3
ton and
2 ton overhead crane had not been operationally tested for a minimum of
5,000
pounds weight which was the combination of both overhead cranes maximum
lifting capacities.
b)sheet metal ring rolling area--an underhung overhead crane with a 300
pound
chain air hoist was not operationally tested.
REFERENCE:ANSI/ASME B30.11-1980, section 11-2.2
Prior to initial use, all new, extensively repaired or modified
equipment shall be tested to assure compliance with this standard.
NOTE:Among other methods of abatement, one feasible and acceptable method
to abate this hazard is to conduct an operational test of the equipment and
maintain a record of the tests.

Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees:
a)cell 3, coil winding department--a nut, used to secure the support
structure for a
jib crane, Number C1011, was missing and other nuts were not tightened to
prevent the support structure from moving as it was being used.
REFERENCE:ANSI/ASME B30.11-1980, section 11-2.3.3
Any hazardous condition disclosed by the inspection requirements of 11-
2.1 shall be corrected before operation of the crane is resumed.
NOTE:Among other methods of abatement, one feasible method to abate this
hazard is
to replace the missing nut and tighten the other lose nuts to secure the
support to
the floor and prevent it from rocking back and forth during operation.

monorail systems shall be inspected by a designated person to assure
compliance with the applicable provisions of this standard.
NOTE:Among other methods of abatement, one feasible and acceptable method

have the hoist and monorail system inspected and continue the inspection
process
on a frequent and periodic schedule.

the conveyor system.
