Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees:
The employees, who operated the Simmons, self-propelled elevating work
platform (number
46125ST), were exposed to a fall hazard.  The guardrail system used across
the access opening
had a broken toprail and was not equipped with a midrail.
NOTE:The attachment bolt for the toprail chain was missing.  The chain
clasp, placed
over the top of the metal post was not able to be closed.  The guardrail
chain was
not capable of withstanding the minimum 200 pounds of pressure.
Among other methods to correct the hazard, one feasible and acceptable
method of abatement
includes the following elements:
1.Maintain and operate the self-propelled elevating work platform in
accordance with ANSI
A92.6-1979.
2.Install an attachment point for the toprail chain that is cable of
withstanding the minimum
200 pounds of pressure.
3.Install a midrail chain that is capable of withstanding the minimum 200
pounds of
pressure.
