Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Serious injuries due to crushing hazards such as but not limited to those brought about by attempts to fix mechanical malfunctions of the oven door (a)Age Oven area; There were inadequate procedures developed and/or implemented for the loading and operation of the Age Oven; (1) Job procedures were not documented, which adequately addressed potential safety hazards such as safe operation of the overhead door to include safety features such as but not limited to the safety dog mechanism and also procedures for minor and major mechanical failures which could reasonably be expected to occur: Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop operational procedures in accordance with safe practices to insure the correct and proper loading and operation of all oven features. Additionally, written safe job procedures shall be established to address all aspects of oven operations, particularly the means for correcting potential mechanical breakdowns.
