Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of fire and explosion due to the release and
ignition of propane:
a)  Employees were exposed to the hazards of fire and explosion while
using a
"Sunbeam" brand, portable liquid propane gas barbecue grill (model 5662D).
On
or about July 17, 1998, employees were exposed to the unexpected release
and
ensuing fire of propane due to the improper use of a gas hose and regulator
assembly in combination with a 40 pound propane cylinder.  At the time of
the
incident, a 40 pound propane gas cylinder was being used in a near
horizonal
position while exerting stress on the regulator and gas hose assembly.
Among others, one acceptable abatement method would be to use and install
a
20 pound gas
propane cylinder in the cart frame so that the bottom ring rests in
notches in the tank support
strap as illustrated on pages 14 and 15 in the Sunbeam assembly
instruction booklet for the
portable liquid propane gas barbecue grill.
