Warehouse - On or about April 30, 1998, fall protection was not provided
for employees
required to stand on a platform (pallet) raised by a fork truck to service
a line hoist.
Employees were exposed to falls up to 12 feet.  A feasible means of
correcting the hazard is
to:
(1)  Use of a safety platform firmly secured to the lifting carriage
and/or forks.
(2)  Restraining means such as rails, chains, etc., shall be in place, or
a body belt with
lanyard shall be worn by the person(s) on the platform.
(3)  An operator shall be in the control position of the truck or
available to operate controls.
