Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to:The hazard of falling or stepping into unguarded hoistway. a)Production Area-First and Second Floor Dumbwaiter:Gates were not interlocked, exposing an employee to a fall of approximately 12 feet to the first floor or being struck by descending elevator. Among other means one feasible and acceptable abatement method is to provide interlock fire resistant doors. Refer to requirement for hoistway door to be equipped with system hoistway door interlocks on page 48 of American Safety Code for elevators, dumbwaiters and escalators A.17.1, 1960 section 111.b. Violation observed on or about 7/15/98.
