Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazard:
a)  Special Finishing Department - On or about 9/14/98, radio controlled
crane #144 was
identified as having an unsafe condition, trolley moved on its own.
Condition was
reported to electronics maintenance.  Crane was returned to service later
that day without
the condition being fully analyzed and corrected.
Abatement certification and documentation required within 10 days after
abatement date.
The certification shall include a statement that abatement is complete,
date and method of
abatement, and states employees and their representatives were informed of
this abatement.
Abatement documentation shall  include documents demonstrating that
abatement
is
complete, such as evidence of the purchase or repair of equipment,
photographic or video
evidence of abatement or other written records.
ABATEMENT NOTE:  Procedures should be documented and implemented to assure
a
systematic approach to reporting hazards noted during inspections of
overhead cranes and other
mobile equipment and the subsequent maintenance activities conducted to
correct hazards.
Reporting of conditions, evaluation of hazards, repair, assurance of
repair, and return to service
should be uniform to assure all conditions reported receive the necessary
attention required to
assure safe operation of the equipment once it is returned to service.
