Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to being struck by moving or falling loads:
a)  Press Line, Around Vertical Conveyor - Employees working
or passing through this area are potentially exposed to being
struck by loads not removed at the second floor level and run
over the top of the conveyor system, while employees working
on the second floor, at the top of the conveyor, are
potentially exposed to being struck by loads as they're lifted
on the conveyor or if they fall from the carrier or are
dropped.
Among other methods, one feasible and acceptable method to
correct this hazard is to install guards or deflectors around
the "back" of the conveyor, or to provide warning signs or
lights, or to provide personnel barriers.  A highly visible
sign showing the safe load limit that can be raised or lowered
should be placed at each loading point of the conveyor.
Start-stop devices should be clearly marked to indicate the
function controlled, and the area about them should be free of
obstructions so they can be seen and reached easily.   The
conveyor could be redesigned so that the carrier comes to a
stop during manual loading or unloading; then the housing
could be equipped with interlocked doors or equivalent safety
barriers at each manual loading or unloading station.  These
doors or barriers would only be able to open when the carrier
has stopped, and the carrier would be unable to move until
they were closed.  Means should be provided to sense
overloads.
Abatement certification required within 10 days after abatement
date.  The certification shall include a statement that abatement
is complete, the date and method of abatement, and state that
employees and their representatives were informed of this
abatement.
