Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling loads and/or falling hoist parts:
a)  Hoist #3 Press 6 - The #3 Loadstar CM 1/2 ton hoist was not kept in
good repair in
that the following deficiencies were noted: (1) The lift failed and
dropped a load near the
operating employee; (2) A preventative maintenance program was not
established: (3)
Periodic inspections were not performed, (4) Load testing was not
conducted with a
written report to confirm the hoists load rating, on or about 3-11-03.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to develop a preventative maintenance program which incorporates all
recommendations outlined
in the manufacturer's manuals with written, signed and dated reports and
also
in accordance with
ANSI B30.16 in order to ensure a safe and proper functioning hoist.
Abatement certification required within 10 days after abatement date.
The certification
shall include a statement that abatement is complete, date and method of
abatement, and
states employees and their representatives were informed of this
abatement.tives
