Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) 430 Prater Avenue, Northlake - On May 15, 1999 an employee was guiding
a suspended
concrete cutter that was lifted while suspended from the bucket of the JCB
front end loader
(Model Number 416) approximately 7 feet above the ground.   The employee
was crushed by
the front end loader.  The concrete cutter was being transported two
blocks down the road. The
bucket blocked the vision of the operator. This condition last existed on
May 15, 1999.
Among other methods, one feasible and useful method to correct this hazard
is to follow the JCB
Operations Manual and Equipment Manufacturer's Institute Wheel
Loader/Tractor Safety Manual
for operators and mechanics.
1. Carry the load approximately 12 inches from the ground level so that
the operator's vision
is not obstructed.
2. Provide and use a crane jib tool that is designed by JCB to be used to
lift the concrete saw
onto a truck or trailer.
Abatement documentation is required for this item in accordance with the
requirements of
29 CFR 1903.19 (d).
