lifting
materials including but not limited to; floor grate.
1)Hoist was not being inspected on a frequent and periodic basis.
2)Support structure and monorail were not designed and installed by a
qualified person.
3)Hoist system was not load tested prior to use.
ABATEMENT NOTES:
Among other methods, one feasible method of abatement is to:
1). Conduct frequent and periodic inspections of the electric hoist for
wear,
deterioration and malfunction as per ASME B30.16-1987.
2). Request a qualified person to inspect the support structure for the
hoist system
as per ASME B30.16-1987.
3). Load test the hoist system as per ASME B30.16-1987.
