recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to A FALL HAZARD:
a)Platform and holding lines, on or about 07/30/03:  Employees daily walk
on
roller conveyors to clear boxes and totes which become "jammed" on the
rollers.  Employees exposed to a fall hazard of approximately nineteen
feet.
Among other methods, several feasible and acceptable methods to correct
these
hazards are to:
1.Prohibit employees from walking or stepping on free turning rollers.
Suitable
means such as tread plates can be used between the rollers as a walking
surface as specified in ASME B20.1-1993.
2.Prohibit employees from working on the holding lines or provide employees
with adequate fall protection.
Abatement certification must be submitted for this item.
