Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard of falling into an elevator shaft: a)Freight / personnel elevator to vertical extrusion tower, on or about 09/19/00: The elevator interlocking devices on the lower level did not prevent the hoistway doors from opening when the elevator was not parked at that level. ABATEMENT NOTES: Among other methods one feasible and acceptable method to abate this hazard is to: Repair or replace interlocks that will prevent door from opening when elevator is not present at that level.

ABATEMENT NOTES: Among other methods, one feasible and acceptable method of abatement is to: Install barrier guard to prevent employee contact with in going nip point.
