Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK BY THE LOAD: a)Throughout the facility, on or about 9/5/03 and 9/12/03: The following hoists had missing or broken safety latches: 1)Annealing Area, 7-1/2 Ton Demac Crane, was missing the safety latch. 2)Bare Wire Area, 1-Ton Coffing Hoist, had a broken safety latch. 3)HVC 427 Area, 1/2-Ton Dayton Hoist, had a broken safety latch. 4)HVC Area, 1/2-Ton Budgit Hoist, had a broken safety latch. 5)Armor Area, 5-Ton Cleveland Tramrail, had a broken safety latch. Among other methods, one feasible method of abatement is to conduct frequent and inspections of the hoists and replace the safety latches on the hooks as needed. ASME B30.16-1993 b)515 Cabler Area, on or about 9/5/03: The support structure for the 2-Ton Dayton hoist had an unknown load capacity rating. Among other methods, one feasible method of abatement is to inspect the support structures for the hoists as per ASME B30.16-1993, to ensure the structure is designed to withstand the loads and forces imposed by the hoist for the rated load and mark the structure accordingly. c)Throughout the facility, on or about 9/12/03: The following below-the-hook lifting devices had unknown load capacity ratings: 1)515 Cabler Area, 2-Ton Dayton Hoist, under-the-hook lifting device with unknown load capacity. 2)Horizontal HVC, Compound Elevator with unknown load capacity. 3)HVC 423 Area, Elevator with unknown load capacity. 4)5-Ton Cleveland Tramrail, under-the-hook lifting device with unknown load capacity.5)Armor Area, 1-Ton Coffing Hoist, under-the-hook lifting device with unknown load capacity. Among other methods, one feasible method of abatement is to conduct frequent and periodic inspections of the lifting device and mark the lifting device with the rated load capacity as per ASME B30.20-1993.
