recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) Employees were exposed to the hazard of being crushed by a falling gas
drilling rig and mast.
The derrick was unevenly stacked with drill stands and high winds were
recorded in the area.
The IDECO drilling rig was rigged up on rain dampened soil and the IDECO
Mast, KM-103-
250-AH, Serial #562, was not guyed per manufacturers recommendations.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be to secure the mast to the ground with guy wires as recommended by
the manufacturer.
