Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees were exposed to the hazard of being crushed by a falling gas drilling rig and mast. The derrick was unevenly stacked with drill stands and high winds were recorded in the area. The IDECO drilling rig was rigged up on rain dampened soil and the IDECO Mast, KM-103- 250-AH, Serial #562, was not guyed per manufacturers recommendations. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to secure the mast to the ground with guy wires as recommended by the manufacturer. ___Abatement certification or documentation is not required for this item. ___Abatement certification is required for this item. _X_Abatement certification and documentation are required for this item. ___Warning tag (or citation copy) required on this equipment before moving.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being trapped on the derrickmans platform in case of a fire or unexpected gas release and having no means of escape. Among other methods, one feasible and acceptable method to correct this hazard would be to install an auxillary means of escape (geronimo line) and escape device from the derrickmans platform to the ground as recommended by the manufacturer. ___Abatement certification or documentation is not required for this item. ___Abatement certification is required for this item. _X_Abatement certification and documentation are required for this item. ___Warning tag (or citation copy) required on this equipment before moving.
