Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of being crushed by the headache post due to the failure of the support beam. Among others, one feasible and acceptable abatement method to correct this hazard would be to obtain the approval of an engineer for all load supporting structures.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of being struck by wire rope or falling loads when the rope or its connections fail while materials are being lifted with the derrick jib and winch truck. Among others, feasible and acceptable abatement methods to correct this hazard would be to: 1. Affix all connecting hardware to lift lines according to manufacturer's recommendations or according to the American Petroleum Institute Recommended Practice 54. 2. Comply with American Petroleum Institute Recommeded Practice 9B to ensure all sheaves and drums are of sufficient size to prevent rope damage.
