Section 5(a)(1) of the Occupational Safety & Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees, in
that, employees were
exposed to:Slipping, tripping and falling hazards when employees were
required to walk on
epoxy coated reinforcing steel as a means of access to work areas 40 to 50
feet away from the
access ladder. The rebar was wet causing it to be extremely slippery.
Among other methods one
feasible and acceptable method would be to secure scaffold plank to the
top of the rebar or
install bed ladders eliminating the slipping and tripping hazards.
Location:Central Artery project C09B1, On top of submersed tunnel section.
