Section 5(a)(1) of the Occupational safety and Health Act of 1970:  the
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to being struck by moving equipment.
a.  On or about March 20, 2000, fall protection was not provided for
employees required to
stand on a platform (pallet) raised by a forklift to add chemicals to a
mixing tank.  Employees
were exposed to falls up to 14 feet.
A feasible means of correcting the hazard is to:
1. Use a safety platform firmly secured to the lifting carriage and/or
forks.
2. Restraining means such as rails, chains, etc., shall be in place or a
body harness with lanyard
shall be worn by the person(s) on the platform.
3. An operator shall be in control position of the truck or available to
operate controls.
So as to conform to the specifications referenced in ANSI "B 56.1. Section
4.17/1988".
