Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by carriers due to lack of an established
maintenance program to
ensure the speed sensor and anti-runaway brake system is operating
correctly:
a- Webb Chassis conveyor #158.  Employees were exposed to being struck by
carriers
moving down the decline of the conveyor due to the speed sensor being set
to high which
failed to engage the anti-runaway brake system causing the carriers to
surge downward.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard includes but are not limited to the following:  a) Employer shall
ensure that the
product being transported on the conveyor is within the rated capacity and
the
rated speed
as referenced in Section 5.1 of American National Standards Institute
(ANSI) B20.1b-
1986.  b) Employer shall conduct routine inspections.  Prevention and
corrective
maintenance programs shall be initiated to insure that all safety features
and devices are
retained and function properly as referenced in Section 5.2(b) of American
National
Standards Institute (ANSI) B20.1-1990 and as stated in The Jervis B. Webb
Company's
"Operation, Maintenance, and Safety Instructions for the GM 325/330
Project" Section
3, page 40 which suggest that a preventative maintenance be performed at
least every 30
days.
