Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees. On L & M Drilling Rig # AFT 2 the derrick ladder located next to the dog house on the south side of the rig was not complete. Section 8.3.4 of API RP 54 states: Each derrick and mast shall be equipped with a fixed ladder providing access from the rig floor to the crown block platform and to each intermediate platform. On L & M Drilling Rig # AFT 2 the chain securing the Kelly hose to the swivel is not properly fastened to the swivel with a clamp. Section 8.7.7 of API RP 54 states: The pump end of the rotary hose should be securely fastened to the derrick or mast by a cable or by a chain clamped to the hose and to the derrick or mast leg. On L & M Drilling Rig # AFT 2 the hinge pins on the tongs do not have cotter keys in place. Section 8.9.6. of API RP54 states: Tongs should be maintained and changed out when excessive wear occurs. All tongs and tong heads including dies should be inspected for size and condition, repaired and parts to be lubricated should be greased prior to every trip.
