derrick ladder was overhanging from vertical for approximately 6-8 rungs.
Overhanging ladders is contrary to API RP 54 - 8.3.3 which states:
Permanently
mounted ladders shall not lean back from vertical.

On L&M Drilling Rig #1, located at the Marbob Coffee Federal #7 well site,
the
tongs used for holding the pipe stands while tripping were excessively
worn by
evidence of 4-5" of play at tong heads and no keeper pins.
Use of excessively worn tongs is contrary to API RP 54 - 8.9.6 which
states:
tongs should be maintained and changed out when excessive wear occurs. All
tongs and tong heads, including dies, should be inspected for size and
condition,
repaired, and parts to be lubricated should be greased prior to every trip.
