employer did not

recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of crushing injuries:
On August 9, 2001, employees working south of the Legging Area assembling
feed bins
were exposed to the hazard of being crushed by a load suspended from the
Grove Crane due
to the fact that the employer failed to establish and follow a program of
frequent and periodic
inspections of the crane system.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
a)establish and follow a program of frequent and periodic inspections of
the
crane system in accordance with ASME B30.5-1994, American Society of
Mechanical Engineers for Mobile and Locomotive Cranes, Section 5-2.1.5.

On August 9, 2001, employees were exposed to the hazard of being crushed
by a load while
assembling feed bins:
a)Employees working south of the Legging Area used a Harrington Hoist,
Serial
Number 027974, attached to a Grove Crane for assembly and welding
activities and the employer failed to establish and follow a program of
frequent
and periodic inspections of the hoist system.
b)Employees working in the Sheller Area used two Harrington Hoists
attached to
the tower for assembly and welding activities and the employer failed to
establish and follow a program of frequent and periodic inspections of the
hoist
system.
c)Employees working in the Assembly Area used a Dayton Hoist attached to
the
beams for assembly and welding activities and the employer failed to
establish
and follow a program of frequent and periodic inspections of the hoist
system.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
a)establish and follow a program of frequent and periodic inspections of
the hoist
system in accordance with ASME B30.16-1998, American Society of
Mechanical Engineers for Overhead Hoists (Underhung), Section 16-2.1.5.

the

a)avoid carrying loads over people in accordance with ANSI B30.16-1998,
American National Standard for Overhead Hoists (Underhung), Section 16-
3.1.3(i).

On August 16, 2001, employees working south of the Legging Area assembling
feed bins
were exposed to the hazard of being crushed by a load suspended from the
Grove Crane due
to the fact that the lifting beam was not marked with the rated load where
it was visible and
that a nameplate or other permanent marking was not affixed on the lifting
beam containing
the manufacturer name and address, serial number, lifter weight and the
rated load.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
a)mark the rated load of the lifting device on the main structure where it
is
visible and affix a nameplate or other permanent marking displaying the
manufacturer's name and address, serial number, lift weight and rated load
in
accordance with ASME B30.20-1999, American Society of Mechanical
Engineers for Below-the-Hook Lifting Devices, Section 20-1.2.1.
