Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
Employees were exposed to the explosive separation of the wheel components
while servicing
tubeless tires on multi-piece (five piece) rims and the employer did not
ensure that the
employees were provided adequate training and protected from the hazards
during the
operation of servicing tire and wheel assemblies.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
a)ensure all air is exhausted from the tire(s) prior to removing any rim
or wheel
components, ensure employees never hammer components while they are
under pressure, provide a restraining device or other protective system in
order to protect employees during inflation, inspection and insertion of
the
valve core, update the process specifications and safe job procedures and
provide training to all employees on the hazards and operating procedures
to
be followed.
Or in the alternative:
29 CFR 1910.177(c)(1):  The employer shall provide a program to train all
employees who
service rim wheels in the hazards involved in servicing those rim wheels
and the safety
procedures to be followed:
Employees were exposed to the explosive separation of the wheel components
while servicing
tubeless tires on multi-piece (five piece) rims and the employer did not
ensure that the
employees were provided adequate training.
29 CFR 1910.177(f)(1):  The employer shall establish a safe operating
procedure
for
servicing multi-piece rim wheels and shall assure that employees are
instructed in and follow
that procedure:
Employees were exposed to the explosive separation of the wheel components
while servicing
tubeless tires on multi-piece (five piece) rims and the employer did not
establish safe
operating procedures for servicing multi-piece rim wheels.
