a.  Receive injuries due to the use of unsafe cat towers supporting the
weigth of 10 metal
pipes of 270 pounds each, 17'6" in lenght, 4" in diameters, schedule 80,
stored at 36"
in height from the floor level.  An employee was working under the pipes
when one of
the towers legs fail, resulting that the pipes slides down hitting the
employee's
head.
Among others method, one feasible and acceptable method to correct this
hazard is to use
safe towers designed to support weight in excess of the load to be applied.
