SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A.  Employer knew or should have known following discussion (on or about
the week of
August 6, 2001) between engineering director and contractor (Amps Welding)
that welder
would be welding a metal clip on top of recovery tank number 245
containing ethylene
glycol which exposed the welder and two helpers to a fire and explosion
hazards.  A feasible
and useful method among other methods to correct this hazard is for plant
management to
designate an individual to authorize the work; advise contractor or
hazards associated with
welding on tank, cease recovery operations; and to verify that tank has
been thoroughly
cleaned of hazardous substances prior to allowing welding operations to
commence in
accordance with NFPA, 51B paragraph 2-2, 2-3, and 3-1.
B.  Employer knew or should have known following discussion between
engineering director
and contractor (Amps Welding) during the week of August 6, 2001 that
contractor's
employees were exposed to fire and explosion hazards when welding clip
onto tank number
245 containing ethylene glycol on or about August 14, 2001.  A feasible
and useful method,
among others to correct this hazard is to inspect the tank and surrounding
work area, and
issue a flame permit in accordance with NFPA, 51B paragraph 3-2 prior to
authorizing
welding activity.
