ORS654.010: The employer did not furnish a place of employment which was safe and healthful for employees, and did not do every other thing reasonably necessary to protect the life, safety, and health of the employees: (a)The primary sub-contractor, Sageland Homes Inc., being the controlling employer, allowed sub-contractors and sub-contractor employees to be exposed to serious hazards by ineffective supervision and training for supervisors regarding hazard recognition for the construction process, as evidenced by: 1)Employees of Todd Franks Construction Inc. were exposed to potential fall hazards up to approximately 11 feet from unguarded edges of the second floor and also an inadequately guarded stair well, without any form of fall protection, located at lot 13, Sherwood Meadows sub division. 2)Employees of Jose Perez Delgado were exposed to potential fall hazards up to approximately 19 feet while walking a 2x6 inch top plate setting trusses, while setting walls on the unguarded second floor, and also the unguarded stairwell, without any form of fall protection, located at lot 15 and 16 of the Sherwood Meadows sub division. Additionally employees were exposed to electrical shock hazards from damaged extension cords that were allowed to be used due to the employer not having and/or lack of enforcement of an Assured Equipment Grounding Program. A SERIOUS REPEAT VIOLATION of Item 01-001 on Citation R1905-044-00, issued May 23, 2000. A SERIOUS REPEAT VIOLATION of Item 01-001 on Citation R1905-050-00, issued May 23, 2000.
