Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employment and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fall hazards from an
elevated personnel
platform:
a)  Big Horn Plastering & 1780 Washington St., Denver, CO 80218: On or
before
9/6/00, the employer did not ensure that a wooden, job-made personnel
platform used
by employees in conjunction with a rough-terrain forklift, was properly
designed,
constructed utilized and maintained. Employees were performing exterior
finish work
on the platform approximately 30 feet above street level.
Among other methods, one feasible and acceptable means of abatement to
correct this
hazard is to design, construct, utilize, and maintain the personnel
platform
in
accordance with ASME/ANSI B56.6.-1992, Safety Standard for Rough Terrain
Forklift Trucks.
