Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of unexpected activation of machinery or
release of stored energy:
At this location, employees working on oil well servicing equipment, such
as, but
not limited to, the Stewart and Stevenson coil tubing unit # 249, were
exposed to
the hazards of unexpected activation of the reel on the units and/or the
release of
the potential energy stored in the tubing that is wound on the reel.
Unexpected
movement of the reel or release of the tubing could cause falls, struck-by
or
caught-between injuries.  Employees assigned to the units were exposed to
the
unexpected activation of the reel or release of the tubing during
operations
such
as placing the tubing in the counter, zeroing the counter, repairing the
injector,
chaining the tubing, cutting the tubing, swapping out reels and other unit
specific
tasks.  The employer did not develop and implement equipment specific,
step-by-
step lockout/tagout procedures for the shutting down, isolating, blocking
and
securing of machines and equipment such as but not limited to, coil tubing
units,
in accordance with,
a.ANSI Z244.1-1982, Lockout/tagout of energy sources - minimum safety
requirements.

c.API RP 54, August 1999, "Recommended Practices for Occupational
Safety and Health for Oil and Gas Well Drilling and Servicing
Operations" and or
d.the Association of Oil Well Servicing Contractors, Third Revision June
1988, "Recommended Safe Procedures and Guidelines for Oil and Gas
Well Servicing".
Among other methods, one feasible and acceptable method to correct this
hazard
is to develop, implement and train employees on written, equipment
specific,
step-by-step lockout/tagout procedures.  Machines or equipment that are
alike in
design and operation may use the same lockout/tagout procedure.
