Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to severe burn injuries from a dust explosion due
flame propagation
through an unrestricted conveying system and other pieces of equipment in
the building:
a.) Particleboard Plant, Milling/Screening, Process Equipment & Conveyors -
Approved means were not provided to prevent flame propagation through the
conveying systems that deliver material to or from pulverizing and/or
sizing
operations.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with the requirements outlined in NFPA 664 ~ Standard
for the
Prevention of Fires and Explosion in Wood Processing and Woodworking
Facilities.
NFPA 664 Sections 10-3 & 11-3.1 states that when material presenting a dust
explosion hazard is delivered to or from the pulverizing operation,
chokes, rotary
valves, explosion suppression systems, or other approved means shall be
provided to
prevent flame propagation through the conveying system.  Devices utilized
to prevent
flame propagation should be installed in accordance with NFPA 69 ~
Standard on
Explosion Prevention Systems and NFPA 68 ~ Guide for Venting of
Deflagrations.ing,

employees were exposed to severe burn injuries from fire because conveying
equipment did
not have facilities to divert burning material to a safe dump area:
a.) Particleboard Plant, Milling/Screening, 60-inch Conveyor - The 60-inch
conveyor
was placed into reverse by manipulating the conveyor's electrical wires in
the motor
control center.  As a result, wood material that could potentially contain
sparks,
smolders, and/or embers was dumped onto the floor. These conditions
existed on or
about February 13, 2001.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with the requirements outlined in NFPA 664 ~ Standard
for the
Prevention of Fires and Explosion in Wood Processing and Woodworking
Facilities.
NFPA 664 Section 11-3.3.1 states that conveying equipment shall have
facilities to
divert burning material from the equipment downstream from the dryer to a
safe
dump area in the event of a fire in the dryer.

the spread of fire and/or explosion between adjacent/adjoining sections of
the plant:
a.) Particleboard Plant, Milling/Screening & Press Lines - Fire protection
features in the
form of fire walls and/or explosion resistant walls were not erected to
prevent the
propagation of fire and/or explosion between the milling/screening area
and press lines.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with the requirements outlined in NFPA 664 ~ Standard
for the
Prevention of Fires and Explosion in Wood Processing and Woodworking
Facilities.
NFPA 664 Section 2-1.2.1 - Passive fire protection features shall be
utilized to prevent
the spread of fire or explosions, or both, between sections of the
facility. Interior walls
erected to isolate dust explosion hazards shall be designed for sufficient
explosion
resistance to preclude damage to these walls before the explosion pressure
can be safely
vented to the outside (NFPA 664 Section 2-2.3*). See NFPA 68, Guide for
Venting of
Deflagrations, for guidance on the strength of relieving and resisting
walls (NFPA 664
Section A-2-2.3).

caused by potential
sources of ignition which included but were not limited to gas fired
heaters and a propane
powered floor sweeper that were located and utilized in Class II locations:
a) Particleboard Plant, Milling/Screening- Gas fired heater(s) were
located on the west
wall above the maintenance department and were not located outside or in a
separate
dust free room or building. Heating for the area was not by an indirect
means, which
was approved for a Class II location.
b.) Particleboard Plant, Milling/Screening - Tennant Sweeper was
designated as LP
and was not designated as EX and was used in atmospheres in which
combustible
wood dust was present.
ABATEMENT NOTES:
a.) Gas Heater - Among other methods, one feasible and acceptable abatement
method to correct the hazard associated with the gas heater is to locate
the gas heater
outside the building and to transfer the heat through duct work.
b.) Tennant Sweeper - Among other methods, one feasible and acceptable
abatement
method to correct the hazard associated with the Tennant Sweeper is to
comply with
the requirements contained in the latest edition of NFPA 505 ~ Fire Safety
Standard
for Powered Industrial Trucks Including Type Designations, Areas of Use,
Conversions, Maintenance, and Operation

that would
automatically divert heat transfer fluid from the utilization equipment
piping to an
emergency loop and/or emergency tank in order to minimize damage/injury
from an
ensuing fire.
