from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
(a)American Ordnance Plant, Warehouse Storage Building #17, Middletown,
Iowa - Grinnell Fire Protection employees were allowed to tape the joystick
handle of a Skyjack,SJIII, model 3220,id# 251800, scissorlift work
platform.
This procedure overrides the safety features of the platform and any
accidental
contact with the joystick would allow the work platform to lunge into
movement.  The careless act put employees in a very hazardous environment
as the employees are installing a sprinkler system in the overhead rafters
of the
storage buildings.
