P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer
did not furnish employment and a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of being struck by hoisted
equipment:
a)  At the worksite, railroad shaker area - Employees were exposed to
struck-by hazards while
performing aggregate unloading work in an area beneath a shaker machine
that was elevated with
an underhung hoist that did not have the wire rope properly installed, on
or about 05/23/01.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is:
1.  When anchoring a wire rope with a wedge socket device, do not attach
("clip") the dead end
of the cable to the live side of the line.
2.  Perform all wire rope replacement/installation work in accordance with
the equipment
operator's manual.
